29 CFR 1910.146(d)(4)(iv): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide personal protective equipment in so far as feasible engineering and work practice controls did not adequately protect employees:  a) Lime Day Bin; on September 15, 2015 and times prior employees were exposed to chemical burns from lime to skin contact in that the employer did not ensure the wearing of hooded Tyvek suits while changing filters to prevent lime/skin contact.

29 CFR 1910.146(f): The entry permit that documents compliance with this section and authorizes entry did not identify the required information listed in (f)(1)-(f)(15):    a) Lime Day Bin; on September 15, 2015 and times prior the employer exposed employees to chemical burns while changing lime filters in that confined space permits did not contain all necessary information including but not limited to:      The name of the permit space to be entered.      A list of the authorized entrants.      A list of the personnel, by name, currently serving as attendants.      The individual, by name, serving as entry supervisor.      A list of the hazards of the permit space to be entered.      The measures used to isolate the permit space and to eliminate or control permit space hazards before entry.      A description of the acceptable entry conditions.      The results of periodic tests to include when the tests were taken.      A list of the rescue and emergency services that can be summoned and the means for summoning these services.      Listing of communication procedures used by authorized entrants and attendants to maintain contact during the entry.      Any other information whose inclusion is necessary, given the circumstances of the particular confined space, in order to ensure employee safety.

29 CFR 1910.146(g)(3): The employer did not provide training that established employee proficiency in the duties required by 29 CFR 1910.146, Permit-required confined spaces, and did not introduce new or revised procedures, as necessary, for compliance:     a) Lime Day bin; on September 15, 2015 and times prior the employer did not provide the training specific to the chemical hazards associated with the Lime Day Bin space for all participants entering the permit required confined space.
