29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a- 2500 Gianna Drive, Glassboro NJ - Lower scaffold: An employee was exposed to fall hazards, in excess of 10 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was without bracing installed 4 feet above the brace to be passed, was missing end stops and did not have a midrail/screen & top rail that extended the entire length of the scaffold's active walking/working surface. On the day of the worksite inspection, this scaffold was not inspected for visible defects, prior to its use, by a competent person. Condition observed on or about 09/12/15. b- 2500 Gianna Drive, Glassboro NJ - Upper scaffold: An employee was exposed to fall hazards, in excess of 22 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was without bracing installed 4 feet above the brace to be passed, was missing end stops and did not have a midrail/screen & top rail that extended the entire length of the scaffold's active walking/working surface. CSHO determined through on site interview of the exposed company employee that he had not received scaffold training from a person qualified in scaffold hazard recognition in order to control and minimize those identified hazards on the day of the worksite inspection. Condition observed on or about 09/12/15. b - 2500 Gianna Drive, Glassboro NJ - Upper Scaffold: An employee was exposed to fall hazards, in excess of 22 feet to the ground surface below, while performing siding installation from a pump jack scaffolds that was missing end stops and a midrail/screen that extended the full distance of the walking/working surface.
