29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a- 191 Second Unit Mould, 932 Lower Ferry Road, West Trenton NJ: On or about 09/29/15 CSHO observed company employees exposed to struck by/caught in between, etc. hazards when areas of the mould unit were unguarded.        b- 196 Conveyor Car, 932 Lower Ferry Road, West Trenton NJ: On or about 09/29/15 CSHO observed company employees exposed to struck by/caught in between, etc. hazards when several areas allowed free, unobstructed access to the unguarded conveyor car.  c - E-4066 saw, 932 Lower Ferry Road, West Trenton NJ: On or about 09/02/15 company employees were exposed to amputation injuries when the piece of equipment was not provided with guarding to prevent an employee from placing any part of their body in the danger zone of the moving blade.

29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:  a- Machine Shop Storage Area - 932 Lower Ferry Road, West Trenton NJ: On or about 09/08/15 CSHO observed company employees exposed to amputation injuries when working/accessing materials next to running/unguarded fan blades of a Nutone wall fan.  The fan blades were in excess of 67 inches above the walking/working surface below.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  a- Pipe Shop - Near Pipe Shop Main Entrance, 932 Lower Ferry Road, West Trenton NJ: On or about 09/08/15 CSHO observed company employees exposed to electrical hazards when utilizing circuit breakers (within breaker boxes) that were not clearly labeled.   b- Pipe Shop - Labeled as Pipe Shop Lighting Panel, 932 Lower Ferry Road, West Trenton NJ: On or about 01/28/16 CSHO observed company employees exposed to electrical hazards when utilizing circuit breakers (within breaker boxes) that were not clearly labeled.       c- Machine Shop - 932 Lower Ferry Road, West Trenton NJ: On or about 09/08/15 CSHO observed company employees exposed to electrical hazards when utilizing circuit breakers (within breaker boxes) that were not clearly labeled.        d- Carpenter Shop - Marked PNL A at the bottom, 932 Lower Ferry Road, West Trenton NJ: On or about 09/08/15 and 01/28/16 CSHO observed company employees exposed to electrical hazards when utilizing circuit breakers (within breaker boxes) that were not clearly labeled.        NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
