29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Skyride maintenance car: On or about September 12, 2015, and times prior to, employees engaged in the maintenance, repair, and inspection of the Skyride system used personal fall arrest systems for fall protection. The employer did not provide any form of relief straps exposing the employees that have undergone a fall arrest to the hazards of suspension trauma and/or orthostatic intolerance.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The Skyride: On or about September 12, 2015, employees were engaged in the replacement of the Skyride cable rollers. The employer did not have a Lockout/Tagout program. This exposed the employees to the hazards of energy sources as, but not limited to, electro-mechanical.
