Safety Data Sheets (SDSs) were not maintained nor readily accessible to employees working on the jobsite located at 415 E Douglas, Wichita, Kansas 67202, for all hazardous chemicals used within the work area.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  Employees working at 415 E Douglas, Wichita, Kansas 67202, were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas.  Employees were exposed to fall hazards from heights greater than 20 feet above concrete in that a training program was not provided to minimize and to recognize the hazards of falling.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  At the jobsite located at 415 E Douglas, Wichita, Kansas 67202, employees were exposed to fall from height hazards in that a fall protection system was not provided.  Employees were leaning over the front edge, approximately 20 feet above the concrete, on the north side of the buildings' roof to caulk and seal roofing materials in place.  Service World Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 911407, citation 2, item number 1 and was affirmed as a final order on 10/29/2013, with respect to a workplace located at 700 Fee Fee Road, Maryland Heights, Missouri 63043.  Service World Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 805601, citation 1, item number 1 and was affirmed as a final order on 03/07/2013, with respect to a workplace located at 306 Locust, Kansas City, Missouri 64108.
