29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) 1165 Dupont Highway, Dover, DE- On or about September 6, 2015 the employer failed to ensure that his employee wore a harness and lanyard attached to the basket while working from an aerial lift.  Both employer and employee were exposed to ejection while hanging a decorative piece/sign approximately 18 feet above ground level.
