29 CFR 1910.178(l)(4)(ii): Refresher training in relevant topics shall be provided to the operator when:  29 CFR 1910.178(l)(4)(ii)(D): The operator is assigned to drive a different type of truck; or   a.  On or about September 10, 2015, located at the Denison University Recycling Facility, employees operated a Crown Narrow-Aisle Reach Forklift in order to load recyclables from a loading dock without the employees being provided with refresher training to cover the safe operation of reach trucks, thereby exposing the employees to the hazards associated with forklift operations.
