29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects.   A) Exterior, Parking lot in bucket: Employees were exposed to head injuries while working in the bucket of a bulldozer below tree trimming activities while not protected with any type of head protection, on or about September 8th, 2015.   B) Exterior, Parking lot: An employee was exposed to head injuries while working in front of and in close proximity to the bucket of a bulldozer while not protected with any type of head protection, on or about September 8th, 2015.
