29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they shall be equipped with such guards when in use.  a.  At the front of the building where a mortar mixer was used to sand and mortar mix to lay over walls, the top of the mixers was not properly guarded thereby exposing employees to a caught-in hazard.

29 CFR 1926.451(g)(4)(xv): Crossbracing is acceptable in place of a midrail when the crossing point of two braces is between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The end points at each upright shall be no more than 48 inches (1.3 m) apart.    a.  At the front of the building where wire was put over the front of the building, the employer only had a cross brace on the back of the scaffold thereby exposing employees to a fall hazard.

29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable:  29 CFR 1926.454(b)(2): The correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question;  a.  On the site where tubular welded frame scaffold was inspected, erected and maintained, the employer did not provide scaffold training for employees thereby exposing employees to a fall hazard.
