29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    A.    Diamond Wipes Int'l Inc. worksite located at 1375 Issac Beal Rd., Bucyrus, Ohio: On or about September 10, 2015, the employer did not ensure employees were protected while operating Machine #209.  An employee accessed the point of operation by reaching under the existing guard attempting to un-jam a line of film. The employee sustained a partial amputation.
