OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing injuries.    a. Site - On or about September 17, 2015, an employee did not operate a Caterpillar skid steer(model 297C S/N CAT 0297CCGCPO1480) safely in that the equipment was operated while an employee was struck and injured while within the immediate proximity.
