29 CFR 1910.26(c)(1): Good safe practices in the use and care of ladder equipment were not employed by the users to get maximum serviceability, safety, and to eliminate unnecessary damage of equipment:  On or about August 1, 2015, the employer provided a portable metal ladder for employees to use for pressure washing windows of the residential home, exposing employees to a fall hazard in that handling the power washing equipment while standing on the ladder could cause workers to lose their balance and create slippery conditions on and around the ladder.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   On or about August 1, 2015, an employee was pressure washing the exterior windows of the residential home when he fell approximately 8 feet from an extension ladder amputating two of his fingers. The employer did not report the amputation injury to OSHA.
