29 CFR 1926.453(b)(2)(v):  A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  On 9/18/2015, employees from The Sullivan Corporation were working at Fresh Thyme located at 2235 Highway K, OFallon MO.  Two employees were attaching sheathing to the side of the building from an aerial lift approximately 25 feet above the ground.  The employees were not wearing body belts with lanyards attached to the booms or baskets.
