29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Instances: a) On or about September 24,2015, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 20 feet, when they were elevated in a large pallet with the use of a forklift (Komatsu FG25T-16 ) prior to receiving written approval from the manufacturer of the equipment. b) On or about September 24,2015, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 20 feet, when they were elevated in a large pallet with the use of an order picker (Raymond Easi R45tt) prior to receiving written approval from the manufacturer of the equipment.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about September 24, 2015, at the above addressed jobsite, the employer did not provide an eye wash for employees exposed to corrosive chemicals such as but not limited to sulfuric acid (38% solution) in the storage warehouse.
