Observed on or about 10/07/2015.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:  (a) At the Plaza-Lincoln Mercury, LLC,  office trailer by the body shop located at 8925 US Hwy. 441, Leesburg, FL 34789 employees were exposed to a shocked by hazard, in that, the electrical cord outer jacket on the outside of the office trailer was damaged. Observed on or about 10/07/2015.
