29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) 301 Staley Die Cutter - Employees were not protected from in-running nip points on a conveyor belt. An employee was injured when their hand became caught in between the rotating tail pulley and the belt while picking up scrap pieces of cardboard, on or about September 15, 2015. No abatement certification or documentation required.
