29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees: (a) Stan Miller, Inc. at 600 S. Ridge St., Breckenridge, CO 80424: On or before September 11, 2015, the exposing employer did not ensure that a stairway, ladder, ramp or other safe means of egress was provided to employees that worked in a trench that was approximately 10 feet in depth, 20 feet in width at the opening, and 30 feet in length. This condition did not provide employees with a safe means of trench egress.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) Stan Miller, Inc. at 600 S. Ridge St., Breckenridge, CO 80424: On or before September 11, 2015, the exposing employer did not ensure that each employee working in a trench that was approximately 10 feet in depth was protected from cave-in. The employer did not comply with the provisions of 29 CFR 1926.652(b)(1)(i), in that the walls of the trench were sloped at an angle steeper than the maximum allowable angle of one and one-half horizontal to one vertical (34 degrees). The classification of the soil was type C. This condition exposed employees to a crushing hazard by excavation wall cave-in.
