OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in the event that unsecured compressed gas cylinders that contained oxygen were knocked over, rolled or fell thereby exposing employees to the hazards from fire (leaking), being struck-by metal fragments (rupturing, or exploding), or being struck-by propelled/pin-wheeling projectiles (cylinder) while working and/or walking by the cylinder storage area. a) South and east wall, 1 Centown Road, Hazleton, PA - Employees worked and walked along exit route walkways where unsecured compressed gas cylinders were stored. In the event that a compressed gas cylinder was knocked over, rolled, or fell, employees were exposed to the hazards of a fire, being struck-by metal fragments, or being struck by a rocketing/pin-wheeling cylinder, as observed on or about October 1, 2015.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of fire, being struck-by metal fragments, or being struck-by propelled/pin-wheeling projectiles when transporting unsecured compressed gas cylinders that contained oxygen. Instance a) South wall, 1 Centown Road, Hazleton, PA - Employees moved full compressed gas cylinders that contained oxygen by hand by either carrying the cylinder or rolling the cylinder on its bottom. In the event that a compressed gas cylinder slipped, rolled, or fell, employees were exposed to the hazards of a fire, being struck-by metal fragments, or being struck by a rocketing/pin-wheeling cylinder, on or about October 1, 2015.

29 CFR 1910.37(a)(2): Exit routes were arranged so that employees would have to travel toward a high hazard area without a path of travel that was effectively shielded from the high hazard area by suitable partitions or other physical barriers. a) North to south exit route along east wall, 1 Centown Road, Beaver Meadows, PA - An exit route walkway that employees could use to evacuate the facility in the event of an emergency had compressed gas cylinders in storage that contained oxygen exposing employees to a potential high hazard area in the path of travel, as observed on or about October 1, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
