29 CFR 1926.403(i)(2)(ii): In locations where electric equipment operating at 600 volts, nominal, or less is exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: On October 01, 2015, a yellow cord was lying on the ground and subjected to physical damage and the cord showed signs of wear and tear, as black electrical tape was used to repair the cords insulation.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.
