29 CFR 1926.602(c)(1)(vi): All industrial trucks in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:   a) Job site, 941 Gibsonia Road, Indianola, PA: On or about October 1, 2015, during construction activities, the employer did not ensue that the Terex fork truck, serial number 98960, met the appropriate precepts of the applicable industry safety standard.  Deficiencies noted during the inspection included, but were not limited to, load chart plates that were illegible, operator restraint systems that were not operational, seats that were damaged, loose wiring being present in the operators cab and a broken rear window above the operator's seat.
