29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    At a metal building construction project, employees working on the ground gathering rain gutter materials were not protected by a protective helmet while working directly below other employees who were performing the gutter installation at higher levels. The employees were exposed to being struck by objects such as tools and materials from a height of approximately 18 feet.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    At this metal building construction project, employees operating a JCB 508c and Genie GTH-842 all-terrain material handlers were lifting and lowering employees installing rain gutters from unapproved work platforms. The platforms in use were not an approved work platforms by the manufacturer for capacity, construction, use, and fall protection. The employees were exposed to falls of approximately 18 feet to the ground below.
