29 CFR 1926.454(c)(3):     The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:    a) Trout Creek Park, Worksite - On or about October 2, 2015, an employee was operating a JLG 800S Aerial Lift when it became unstable and overturned. The employer had not provided specific training on this type of scaffold.    Among other feasible means of abatement, operators must only use this type of aerial lift in a manner as found in the JLG 800A/AJ Operator's Manual under 1) Section 4, Machine Operation, Figure 4-1, which states "Machine will upset in this direction if overloaded or operated on an out of level surface".  Additional operational data related to this condition may be found in the JLG Ground Support Training Manual on pages 42 and 43.
