29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:   (a) At the residential home, protective helmets were not used to protect workers from the hazard of falling or flying tools and materials as a result of employees walking and working below workers above them.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Residential Home , during roofing construction operations, at least four (4) employees, were not protected from a fall hazard of approximately 11-feet.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) Residential Home, employees performing framing operations, employees climbed 11-feet from the lower surface level to access the upper working surface level. Employees were exposed to the hazards of falling to the lower level.
