29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: a. On or about October 2, 2015, at the 1st floor, a duplex outlet did not have a faceplate and exposes subcontractor employees to electrical hazards when using the outlet to power portable equipment. b. On or about October 2, 2015, at the 4th floor, a duplex outlet did not have a faceplate and exposes subcontractor employees to electrical hazards when using the outlet to power portable equipment. c. On or about October 2, 2015, at the 2nd floor hall, electrical junction boxes that powered light bulbs were not provided with covers which exposes employees to electrical hazards.
