29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about October 7, 2015, at the worksite located on Red Wolfe Boulevard & Apache Drive in Jonesboro, Arkansas, employees working atop a storage container building engaged in metal stud framing and plywood wall installation tasks were exposed to unprotected falls of greater than 6 feet.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about October 7, 2015, at the worksite located on Red Wolfe Boulevard & Apache Drive in Jonesboro, Arkansas, employees used a Louisville step ladder that was folded and leaned against the storage container building wall to access and descend from the top of the storage container.  This condition exposed employees to falls from elevation.
