29 CFR 1926.102(a)(1): Employee shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.   At the jobsite located at 8100 Drury LN, Mission Hills, KS; an employee engaged in residential roofing activities using a pneumatic nail gun and was exposed to struck by hazards in that the employee was not wearing eye protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section.  At the jobsite located at 8100 Drury LN, Mission Hills, KS; employees engaged in residential roofing activities on a 6/12 pitch roof were exposed to fall hazards of approximately 12 feet from roof eave to the ground, in that fall protection was not used.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite located at 8100 Drury LN Mission Hills, KS; employees engaged in residential roofing activities  at heights greater than 6 feet on a 6/12 pitch roof were not trained to recognize and minimize the hazards associated with falls.
