(a) 8083 Princess Path, Liverpool, NY 13088 - On or about 10/8/15 - Employees were working on a 4/12 pitch roof above six feet in elevation without using proper fall protection.      Home Advantage Plus, LLC. was previously cited for a violation of this occupational safety and health standard 29CFR1926.501(b)(13), which was contained in OSHA inspection number 942663, citation number  1 item number 1 and was affirmed as a final order on 10/31/13 with respect to a workplace located at 8496 Henry Clay Blvd, North Syracuse, NY.
