29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  On or about October 8, 2015, Hirschvogel, Inc., South Third Street, Columbus, Ohio establishment: For the OSHA 300 log, the employer had not included all the information in columns E and F as required for each recordable injury or illness, including, but not limited to, calendar year 2015 Case Numbers 21, 22, 23 and 24; none of which included the location where the event occurred or the object/substance that directly injured or made the employee ill. Additionally, the calendar year 2015 OSHA 300 log had case numbers 19 and 20 were without any column (F) information entry.
