29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    a)Body Shop - An employee used a tight fitting North full face respirator with a dual cartridge system on a volunatary basis while working with chemicals and the employer did not have him medically evaluated to ensure the respirator did not present a health hazard to the employee.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:  a) Warehouse - The Indco stand-up type mixer had a pulley drive system at approximately  6 foot, 2 inches from the floor that had no guarding to prevent accidental contact with the belt pulley system, exposing employees to amputation hazards.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   a)  The facility - Employees were required to work with chemicals such as, but not limited to, Polymeric MDI and Mineral spirits, and the employer did not develop and implement a written Hazard communication program.
