29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a) On or about October 8, 2015 at worksite 1503-1513 Levingston Court Kansas City, Missouri.-Employees were performing roofing operations at an elevated height without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to an 18-22 ft. fall hazard to the concrete surface below.    Abatement certification for this violation is not required.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On or about October 8, 2015 at worksite 1503-1513 Levingston Court Kansas City, Missouri.-Employees   Employees were accessing a working surface on a steep pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.     Abatement certification for this violation is not required.
