OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using an automotive lift:  (a)  On or about October 8, 2015, and at times prior the employer did not ensure that employees        were safe from crushing and struck-by hazards in that the following lift was noted with deficiencies:      (1)  Rotary Lift, model SDOA9-202, serial #SDCOOD0051, did not have the locking mechanism locked            into place while the lift was raised and in use.      (2) The employer did not document that all lift users were trained to use the lifts. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  On or about October 8, 2015 ,employees were exposed to injuries from flying parts while using a Central Hydraulics 20 ton shop press where the employer failed to ensure guards were in place prior to use to protect employees.  (b)  On or about October 8, 2015 ,employees were exposed to injuries from flying parts while using a Hunter Brake Lathe where the employer failed to ensure guards were in place prior to use to protect employees.
