29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 10/8/2015 at 2255 New Point Parkway, Lawrenceville, GA, Roof: Two (2) workers were observed performing Thermoplastic Polyolefin (TPO) membrane installation from the cap of the parapet wall, exposed to falls of up to 34 feet to the lower level where the workers were not tied off.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: On or about 10/8/2015 at 2255 New Point Parkway, Lawrenceville, GA; two workers were observed performing Thermoplastic Polyolefin (TPO) membrane installation along the edge of the building and were exposed to fall hazards of up to 7.16 feet (86 inches) to the lower level where the Werner 6 foot stepladder was used in a closed configuration and the membrane was wet, allowing for potential slipping of the improperly used ladder.
