29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers. On or about October 7, 2015, on all floors 2nd thru 7th floor penetrations 2" wide had been drilled to run piping for all vent, water, and waste lines. Employees working below were exposed to struck by hazard from tools or debris falling through the holes to the lower level.
