29 CFR 1926.652(g)(2): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of a shield system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the shield:    On or about October 7, 2015, at 1625 Mt. Vernon Road, Dunwoody, Georgia:    Two employees performing utility construction work were exposed to cave-in hazards when they worked inside of a section of an approximately 48-foot long, 4-foot wide, and 6-foot deep excavation that had a trench box installed approximately 5-feet from the bottom of the excavation.

The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that:    On or about October 7, 2015, at 1625 Mt. Vernon Road, Dunwoody, Georgia:    Two employees performing utility construction work were exposed to cave-in hazards while working inside a section of an approximately 48-foot long, 4-foot wide, 6-foot deep excavation that did not have a protective system installed.
