29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  a) Along the interior north center side first floor of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts, from a 4" X 4" metal junction box with a broken ground fault interrupter and exposed electrical parts while plugging in equipment into the electrical box.   b) Along the interior north center side second floor of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts, from a 4" X 4" metal junction box with a broken ground fault interrupter and exposed electrical parts while plugging in equipment into the electrical box.  c) Along the interior south center side second floor of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts, from a 4" X 4" metal junction box with a broken ground fault interrupter and exposed electrical parts while plugging in equipment into the electrical box.    d) Along the interior south center side third floor of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts, from a 4" X 4" metal junction box with a broken ground fault interrupter and exposed electrical parts while plugging in equipment into the electrical box.

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed; and conductors entering boxes, cabinets, and fittings were not protected from abrasion:   a) Along the interior north center side of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts from an open and unused circuit breaker blanks, from a Square D electrical circuit box category number HOMC24UC, while accessing an electrical panel during electrical installation of temporary wiring.  b) Along the exterior east side of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts, from a Square D electrical circuit box category number 4O265-761-01, that had an energized Romex wire conductor running through an opening without any type of abrasion protection along the bottom of the box. while accessing an electrical panel during electrical installation of temporary wiring.   c) Along the exterior south west side of the residential construction building, on or about October 07, 2015, employees were exposed to electrical shock hazards of 120 volts, from a Square D electrical circuit box category number 4O273-834-02, with an opening without a knockout seal along the bottom of the box while accessing an electrical panel during electrical installation of temporary wiring.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, or fittings were not provided with covers and in energized installations each outlet box did not have a cover, faceplate, or fixture canopy:  Along the interior south center side third floor of the residential construction building, on or about October 07, 2015, employees conducting electrical installation were exposed to electrical hazards of 120 volts from an uncovered outlet source  with exposed live parts.
