29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    Along the south corner exterior roof of the construction building, on or about October 06, 2015, an employee in a JLG aerial lift model 600S without a lanyard was exposed to the hazards of fall while conducting finishing work from heights greater than 6 feet above the ground below.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems:     Along the south side awning roof on top of a 4 level residential building, on or about October 10, 2015, an employee was exposed to fall hazards while conducting finishing work on top of a awning roof at heights greater than 6 feet above the ground level below without having any type of fall protection in place.
