On or about 13 October 2015, and at times prior thereto, two re-locatable power taps were mounted to a metal rack stand and a metal wheeled table in a manner not in accordance with the manufacturer's intended use, exposing the personnel working within the Austin Plasma Lab (APL) to an electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  On or about 13 October 2015, and at times prior thereto, with the Austin Plasma Lab (APL), a Belkin re-locatable power tap was connected to an outlet near the ceiling with the power cable running over a U channel and the unit hanging from the metal channel without any supporting device or mechanism causing tension upon the terminal connections within the unit housing, exposing personnel working in the APL to an electrical hazard.
