29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented: At the construction site, employees were installing shingles without fall protection exposed to falling more than 18 feet to the lower level. Jose Huerta was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 905450, Citation Number 1, Item number 1 and was affirmed as a final order on June 06, 2013, with respect to a workplace located at 8311 Kara Creek Road, Little Elm, Texas 75098.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the construction site, employees used ladders that were not extended at least 3 feet above the upper landing to access the working area exposed to falling more than 18-feet from the top plates.
