29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     (a) On or about October 14, 2015, By George Construction, LLC. did not ensure that eye and face protection was used by employees when machines or operations presented potential eye or face injury. Employees were observed nailing shingles on a residential roof, using a pneumatic gun, without any eye or face protection.     Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     (a) On or about October 14, 2015, By George Construction, LLC. did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees were exposed to fall hazards of up to 13 feet while engaged in roofing activities of a residential building with no fall protection in place.
