29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Employees working in the concrete assembly building were exposed to electrical shock from the 110 volt AC electrical panel located in the northwest corner of the building. The unused openings in the breaker panel were not covered.

29 CFR 1910.305(d)(2): Panelboards mounted in cabinets, cutout boxes, or enclosures designed for the purpose and were not dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons: Employees working in the concrete assembly building were exposed to electrical shock hazards in that the 220 volt AC main electrical panel was missing the dead front cover exposing the energized electrical wiring and bus work.
