29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  1) On October 14, 2015, and at prior times, employees were exposed to cave in hazards in the following instances:    a) while working in a trench on the north side of the site that was in excess of 7 deep and had walls that were not adequately sloped.     b) while working in a trench on the north side of the site that was in excess of 5 deep and had inadequate benching.
