29 CFR 1910.110(f)(2)(i):  Containers in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons.  On 10/15/2015, Rock Bottom Wholesale Inc., located at 1120 Howard Street, St. Louis, MO, had not ensured LPG tanks used to fuel powered industrial trucks were stored or located so as to minimize exposure to physical damage.  Ten tanks were stored near the entrance of an overhead door utilized by forklift operators to move merchandise.

29 CFR 1910.178(l)(1)(ii):  Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training.  On 10/15/2015, Rock Bottom Wholesale Inc., located at 1120 Howard Street, St. Louis, MO, had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.  The warehouse supervisor and two employees were operating powered industrial trucks without required training.  Both employees were not wearing seatbelts while operating the powered industrial trucks.

29 CFR 1910.178(p)(1):   A powered industrial truck was found to be in need of repair, defective and unsafe and the truck was not taken out of service until it had been restored to safe operating condition.  On 10/15/2015, employees working at Rock Bottom Wholesale Inc, located at 1120 Howard Street, St. Louis, MO, were operating powered industrial trucks, which were in need of repair, defective and unsafe.  The following defective forklifts were on site:  1.  Toyota Forklift, Model 42-5FG25, / Serial # 78780, was missing pads on the pedals and had the LPG tank improperly installed.  2.  Hyundai Forklift, Model HLF-15C-5 / Serial # FN0110317, was missing pads on the pedals, tank hold-down straps and head lights.  The forklift had a damaged seat and improperly installed LPG tank.

29 CFR 1910.178(q)(7):   Industrial trucks were not examined before being placed in service, and were not removed from service when the examination showed any condition adversely affecting the safety of the vehicle. Such examination was not made at least daily.   On 10/15/2015, employees working at Rock Bottom Wholesale Inc., located at 1120 Howard Street, St. Louis, MO, were operating two defective powered industrial trucks.  The powered industrial trucks were not examined before being placed in service.    Such examination was not made at least daily. The following defective forklifts were on site:  1.
