On 10/15/2015, employees working at Rock Bottom Wholesale Inc, located at 1120 Howard Street, St. Louis, MO, were operating powered industrial trucks, which were in need of repair, defective and unsafe.  The following defective forklifts were on site:  1.  Toyota Forklift, Model 42-5FG25, / Serial # 78780, was missing pads on the pedals and had the LPG tank improperly installed.  2.  Hyundai Forklift, Model HLF-15C-5 / Serial # FN0110317, was missing pads on the pedals, tank hold-down straps and head lights.  The forklift had a damaged seat and improperly installed LPG tank.
