29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On the front porch of the building, employees who were working on the porch while an employee was nailing joists to the porch roof were not wearing hard hats, exposing employees to being struck by falling materials and/or tools.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On the north side of the building on the porch roof, an employee was installing roof joists without using any type of fall protection and was exposed to an unprotected fall hazard of more than 6 feet to the ground below.
