29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a). Worksite located at 981 S. 291 Highway, Lee's Summit, Missouri- Employees were exposed to a shock or electrocution hazard in that the employer had removed the end of the plug and re-wired to another male end in which the inner wires were exposed as well as the cord was not provided with a grounding pin.
