29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  At the jobsite, on the roof, on or about 10/15/15:  Workers were working on a low slope roof deck, 14 feet from grade, in close proximity to the edge, without fall protection of any kind.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At the jobsite, on the roof, on or about 10/15/15:  Workers were accessing the roof deck using an extension ladder where that ladder did not extend at least 3 feet above the upper landing.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.  a)  At the jobsite, on the roof, on or about 10/15/15:  Workers were using power tools energized by a portable generator with a connection through a portable power tap that was not designed for construction purposes.
