29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:      (a) On or about October 14th, 2015, at a job site located at 3350 Gillham Road in Kansas City, MO, an employee was exposed to potential electrical shock hazards in that the sole source of electrical power available to him was provided by a three-outlet extension cord (a type of temporary branch circuit) which had been plugged into a permanent outlet within the building and which was unprotected by a properly functioning GFCI.  The employer had not instituted an assured equipment grounding program.        Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):      (a) On or about October 14th, 2015, at a job site located at 3350 Gillham Road in Kansas City, MO, an employee was exposed to a serious fall hazard while working on a scaffold platform that was not fully decked between the front uprights and the guardrail supports as required by standard.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:      (a) On or about October 14th, 2015, at a job site located at 3350 Gillham Road in Kansas City, MO, an employee was exposed to serious fall and scaffold collapse hazards while working from a scaffold with a height to base width ratio greater than four to one (4:1) which was not restrained from tipping.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:      (a) On or about October 14th, 2015, at a job site located at 3350 Gillham Road in Kansas City, MO, an employee was exposed to a serious fall hazard while working from a scaffold platform which was more than 10 feet above the lower surface.  The scaffold platform lacked any type of a guardrail system, and no alternative fall protection measures were in place or in use at the time of the inspection.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:      (a) On or about October 14th, 2015, at a job site located at 3350 Gillham Road in Kansas City, MO, an employee was observed to be working from a scaffold platform.  The employee had not been trained in recognizing the hazards of working from a scaffold, and had not been trained in the appropriate measures to take to control or minimize those hazards.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
