29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    a) Jobsite: On October 9, 2015 and a tiimes prior thereto an employee did not recognize or acknowledge the hazard of working outside of the cave-in protective system that was in place.       Note) Abatement certification IS required for this item.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  a) Jobsite: On October 9, 2015, and at times prior thereto cave in protection was not provided for employees who performed pipe connection activities outside of a concrete irrigation vault.  b) Jobsite: On October 9, 2015, and at times prior thereto cave in protection was not provided for employees while exiting the excavation using an extension ladder.    Note) Certification of corrective action IS required for this item.
