29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Top Level, Left Side Rear Corner, - Two workers were observed standing on the top plate of a wood stud wall frame not protected from falling approximately twenty five feet to the ground and scrap material below, on or about 10/15/2015. b) Top Level, Right Side Rear Corner and Center Edge - One worker was observed standing on the top plate of a wood stud not protected from falling approximately twenty five feet to the ground and scrap material below, on or about 10/15/2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Right Side Rear of Structure - An extension ladder used by the workers to gain access to the top working level of the structure extended approximately twelve inches past the landing surface on or about 10/15/2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
