29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On or about 10/19/15, on the south side of a house at 1632 Orchard Park Road, West Seneca, NY: Employees were on the "pik" of a ladder jack scaffold while engaged in transporting roofing materials and accessing portions of the roof.  The employees were exposed to a fall of approx. 14 ft. to the ground.  No personal fall arrest system or other fall protection system was utilized.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about 10/19/15, on the south side of a house at 1632 Orchard Park Road, West Seneca, NY:  Three employees were on a steep roof with a pitch of approx.  12 in 12 and engaged in roofing work including but not limited to tearing off old shingles and installing new shingles.  The employees were exposed to a fall of approx. 15 ft. to the ground.  No personal fall arrest system or other fall protection system was utilized.      NO ABATEMENT CERTIFICATION REQUIRED
