29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a) Location - Merrimac Street, Methuen, ma: On or about 10/19/15, employees were exposed to fall hazards when employees working on a roof were exposed to more than a six foot free fall distance.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder:  (a) Location - Merrimac Street, Methuen, MA: On or about 10/19/15, employees were exposed to falls when using a portable ladder that was positioned correctly, the ladder was not positioned at four to one angle for use.
